- You believe that your personal details and medical information were compromised in the Heritage Provider Network 2022 data breach.
- You received a notice from the settlement administrator with the Unique ID and PIN.
What will be the payout amount?
The payout from this settlement is going to vary from one another who will file a claim form. The payment amount is going to depend on filing type; if you claim out-of-pocket expenses by providing documents such as receipts, bank statements, and police reports, you can get a maximum payment of $10,000.
You can also get the compensation for the spent time; note that it can be claimed for up to 7 hours with an hourly rate of $30. Note that if you don’t provide the required document, you can claim it for a maximum of 3 hours. Those who don’t claim the out-of-pocket expenses can expect the payout range to be $68.72 to $357.97.
The cap of the out-of-pocket expenses is $2 million, and for the spend-hour compensation, it is only $1 million. The fund will be distributed among the other claimants after deducting attorney fees and administration costs.
Along with the payout, class members will also get three years of free credit monitoring and identity protection services, which includes dark web monitoring and up to $1 million in identity theft insurance.

- Go to the portal by visiting regalmedicalsettlement.com/.
- Click on the ‘Submit A Claim’ button and go to the next page.
- Fill in your Unique ID and PIN, and click on the Login button.
- Provide details, and choose the claim form type to claim payout.
- Choose to get the payment via PayPal, Venmo, Zelle, virtual prepaid card, or paper check.
- Review all the details you have provided in your claim form, and click the Submit button to file it.
Offline:
- Download the claim form from the official settlement website.
- Fill out your name, contact information, and Claim ID and PIN.
- Attach proof if you’re claiming out-of-pocket losses.
- Sign and date the claim form before mailing.

What about the payout?
