Thousands of the customers of the Frontier Communications, telecommunications company which provides internet, television and phone services, were affected by the 2024 data breach. Due to the incident many customers lost money, because their personal details such as name, SSN, address, etc, were accessed by the hacker.

The payout will be distributed among the affected individuals after deducting the attorney fee and administration cost. In April 2024, Frontier Communications experienced a data breach, and between May to June 2024, it began an internal investigation to identify the affected individuals. What are the requirements to submit a claim form?
